Patronage care in the municipality of Troyan

The project "The Patronage Care in the Municipality of Troyan" has a duration of 17 months and will provide mobile integrated health and social services for 12 months to 104 people from the target groups: people with disabilities and their families, and elderly people over 65 with limitations or incapablilities of self-service from Troyan and the near 21 villages. Aims: 1. Improving the quality of life and the opportunities for social inclusion of people with disabilities and the elderly in Troyan Municipality by providing a network of services in the home environment and building sufficient material and personnel capacity for their provision. 2. Establishing a working model for patronage care for elderly people and people with disabilities, including people with chronic diseases and permanent disabilities, through the provision of hourly mobile integrated health and social services in their homes. 3. Providing long-term care for the elderly and people with disabilities through continuity and improvement of implemented programs and projects under OP HRD and CMD to finance social services in the home environment. Activities: 1. Provision of hourly mobile integrated health and social services for disabled and elderly people in the territory of Troyan Municipality. 2. Acquisition of a transport vehicle for mobile work, furniture and office equipment for the mobile health and social care management team in the premises of the selected SGEI contractor. 3. Staff training and supervision as well as mandatory organisational activities and project management, and information and communication activities. The measures for achieving the results are: selection of the target groups; assignment of the patronage, such as SGEI, according to the EC decision of 20 December 2011; buying a car for mobile work; equipment and furnishing of a service management office; training of the contractors under the MH program, provision of services.
