The main priorities in the local policy for the development of social services in the Municipality of Zlatograd and the Municipality of Chepelare are expansion the range of social services offered to children and people with disabilities and their families, opening of new social services, increasing the capacity of the existing ones and improving the quality of the offered social services. Herewith project proposal will improve the life quality and the opportunities for social inclusion of people with disabilities and the elderly people from the municipalities of Zlatograd and Chepelare. The developed project for patronage care will provide hourly mobile integrated health and social services in people’s home in order to provide consumers with the opportunity to preserve their self - dependence and lifestyle. The project will also build up relevant staffing capacities for their delivery and management. The staff who will provide the services will be trained and supervised during the implementation of the project activities. Two vehicles for the provision of health and social services in the Municipality of Zlatograd and the Municipality of Chepelare will be purchased under the project. This material resource is of great importance for providing the mobility of the service, ensuring access to consumers from small settlements in both municipalities. Because of their location and demographic characteristics, they are characterized by remoteness from health and social infrastructure, difficult transport communications, declining, mostly elderly population. A joint team of experts from both municipalities will be set up to organize and manage the project, which will ensure the quality of the project implementation and the coordination between the partners.
