Patronage care for elderly people and persons with assurances in the Municipality of Popovo.

This project proposal aims at providing support for the social inclusion of vulnerable groups of people by providing a network of services in the home environment and building appropriate (physical and staff) capacity for their provision in the Municipality of Popovo. The implementation of the project will create the appropriate conditions for building a model for patronage care for the elderly and the disabled, with chronic diseases and permanent disabilities in order to provide hourly mobile integrated health and social services in their homes on the territory of the Municipality of Popovo and in the sphere of the social economy. The provision of hourly mobile integrated health and social services is carried out in accordance with the program / methodology / developed by the Ministry of Health to provide patronage care and service quality standards within Component 1 under the Patronage Care for the Elderly and Disabled The following activities will be implemented within the project: 1. Provide patronage care for a period of 12 months to 71 individuals in the target group; Providing psychological support, counseling; Training and Supervision of Staff Expected Results: The project will provide an upgrade of the established model for the provision of hourly mobile integrated health and social services for the elderly by increasing the results of the project "Home care for an independent and dignified life" for a period of up to 12 months in the territory of Popovo Municipality. Through the development and implementation of the project, social impact will be achieved by supporting vulnerable groups of people on the territory of Popovo Municipality. The objectives of social inclusion policies at municipal, regional and national level covering all vulnerable communities and individuals in the target group will be achieved.
