Patronage care for the elderly and disabled people in Mezdra Municipality

The implementation of the project "Patronage of elderly people and disabled people in the Mezdra Municipality" will provide support to disabled people and persons over 65 who have difficulties in their day-to-day care, suffer from serious concomitant health problems. A significant percentage of the elderly can be assigned to the group of people with disabilities Denmark. In addition to home care, these people will be provided with specialized medical care in their homes-building a model for patronage care for elderly people at home. The project is in line with Measure 2 (Operation 6): Providing patronage care by providing hourly mobile integrated health and social services for disabled people in need and implementing the Action Plan for the period 2018-2021 for the implementation of the National long-term care strategy adopted by CMD No 28 / 19.01.2018 and is part of the overall process of deinstitutionalization of the care for the disabled and the elderly and the establishment of alternative community support through the provision of quality and affordable services for -term care. Through the implementation of the project activities, the quality of life and opportunities for social inclusion of people with disabilities and the elderly will be improved by providing a network of services in a home environment. With the implementation of the project, the Municipality of Mezdra will support the achievement of the national targets set in the National Long-Term Care Strategy. The provision of hourly mobile integrated health and social services is being implemented in accordance with the program / methodology / developed by the Ministry of Health to provide patronage care and service quality standards within Component 1 under the Patronage Care for the Elderly and Disabled. Medical specialists and professionals in the field of social services involved in the provision of patronage will undergo specialized training.
