Patronage care in the municipality of Asenovgrad

The main objective of the project proposal and to build a model for hourly mobile integrated health and social services - a patronage of disabled and elderly people in Asenovgrad Municipality for improving the quality of life and their opportunities for social inclusion. Asenovgrad municipality will provide suitable premises for repair and equipment and furnishing. A car will be purchased to ensure the mobility of the service. A team of specialists (nurses, housekeepers, social workers, etc.) will be hired to provide various health and social support activities. Support is provided to staff directly involved in the provision of health and social services - training and supervision. The implementation of the activities will be covered and access to long-term mobile and integrated health and social care will be provided, according to the specific needs of 168 persons from the target groups, namely: Adults over 65 with limited or inability to self-service; People with disabilities and their families. They will also receive psychological support and counseling. Patronage will be provided for 12 months within the project, and after its completion, the municipality will seek other funding opportunities. The project proposal was developed in accordance with Measure 2 (Operation 6): Providing patronage care by providing hourly mobile integrated health and social services for the disabled and the elderly in need of the Action Plan for the period 2018-2021 for implementation of the National Long-Term Care Strategy adopted by Council of Ministers No 28 / 19.01.2018 and is part of the overall process of deinstitutionalization of the care for the elderly and the establishment of alternative community support through the provision of achestveni and affordable services to long term care.
