Patronage care for elderly people and people with disabilities in the Municipality of Tran and the Municipality of Zemen

The project is implemented on the territory of the two municipalities of Trun and Zemen with similar demographic and infrastructure problems. Municipalities are characterized by remoteness from large hospitals and difficult access to medical services due to limited transport.Project target groups are people with permanent disabilities and adults over 65 years of age, including chronic illnesses that do not cope without support.Target groups will include people who do not have close relatives able to help them,do not use social services in the community. These users due to poverty and transport restrictions are without access to social and health services in the municipal center of the respective municipality. Integrated social-health services will be provided after a precise assessment of the needs of each individual. An opportunity is created for some users of the project to receive social services, medical services, psychological assistance for self-management, as well as help with buying medicines, food products and others for the household, certification of health books; administrative services; advice on rights and obligations.The project provides a decent life for the participants by providing integrated hourly services in a home environment and facilitated access to social and health services.A mobile team will work on the ground at the home of consumers with regard to the prevention of life-threatening illnesses, health promotion, motivational and psychological support; medical assistance for disabled people who are without access to social and health services in the community.The project will work with a team of specialists, who will schedule visits for consultancy, social and health services of elderly persons and persons with disabilities in the settlements of the municipalities. For on-the-ground work for mobile teams in both municipalities, appropriate equipment and cars will be delivered to reach the villages furthest from the municipal centers.
