Patronage of elderly people and people with disabilities in Samokov

With the implementation of the project, conditions will be created to improve the quality of life of people with disabilities and the elderly, including chronic diseases and permanent disabilities, by providing integrated health and social services in their homes, which will reduce the risk of social exclusion . The services to be provided will be tailored to the individual needs of each user. This will enable them to feel equal and meaningful, and this will in turn follow the basic principles of European society - social work.  The project will employ medical professionals, a rehabilitator and a specialist in social work and counseling, which will help the consumers in their everyday life, according to the individual needs of each one. Most problems of people with disabilities, the elderly and people with chronic illnesses are due to serious health problems and the inability to perform their day-to-day activities. They often live in social isolation and lose sense of significance. The implementation of the project will also help to achieve the national targets set in the National Long-Term Care Strategy. In this way, elderly people with chronic illness and permanent disability will receive medical care psychological assistance in their homes by building a model of patronage care for elderly people at home.
