Patronage of elderly and disabled people in Polski Trambesh municipality

Many people in Polski Trambesh, due to poor health and / or elderly age, are partially or totally incapable of self-service. They have daily care needs, improved access to social and medical services, and social networking. In order to meet these needs, Polski Trambesh Municipality has developed this project. It aims at providing accessible, high quality services for people with disabilities and the elderly by providing patronage care in a home environment. With the implementation of this project proposal, the volume and diversity of social services provided in the municipality of Polski Trambesh will be enhanced, the quality of life and the opportunities for social inclusion of people with disabilities and the elderly will be improved, thus protecting the public interest and respond to the needs of the population. It is envisaged to develop a model of patronage care for the target groups in order to provide hourly mobile integrated social and health services in their homes, the services being provided by medical specialists, psychologist, social assistants, assistant social activities. All medical specialists and persons involved in the provision of patronage care should undergo training under a program developed by the Ministry of Health. Transport will be used for the purpose of the project, thus ensuring the mobility of professionals providing social-health services and faster mobility to the homes of consumers. Project target groups are: - People with disabilities and their families; - Older people over 65 with limited or inability to self-service.
