The health-demographic situation of the population in the Republic of Bulgaria is characterized by aging, chronic morbidity , and disability. The limited financial resources of adults, people with disabilities and their families; as well single members of a household, intensify the need to develop alternative methods of health and social care service delivery. Our project aims to improve the quality of life and prevent the social isolation of adults over 65-years-old with difficulties in living independent lives as well as individuals with disabilities and their families. The project with deliver the above aims by: • Improving the capacity and expertise of the Municipality of Varna for providing integrated social and health care services for adults and persons with disabilities and expand the network of accessibility and quality services in the home. This project is consistent with the National Strategy for long-term care and the Action Plan for the period 2018-2021; • Enabling an hourly, mobile and integrated health and social care service in a service user’s home; • Consulting service users and their relatives on medical and social problems; • Establishing good relationships with general practitioners and exchanging information with other professionals of a service user’s care status. The outcomes of the planned activities will be the prevention of hospitalization or by reducing the time spent in hospital, as well as the reduction of the number of adults and people with disabilities placed in institutions.
