Patronage care for the elderly and disabled - Silistra

Through the implementation of the project proposal there will be developed a model for community care for elderly people and persons with disabilities, including people with chronic illnesses and permanent disabilities to provide hourly mobile integrated health and social services in their homes. The aim of the project proposal is to improve the quality of life and the opportunities for social inclusion of people with disabilities and the elderly by providing a network of services in the home environment as well as building adequate capacity for their provision. Hourly mobile integrated health and social services for people with disabilities and elderly people in the municipality of Silistra will be provided within the framework of the Project as well as Staff training and supervision. An appropriate means of transport will be purchased to carry out the mobile work. 220 persons from the target groups will be covered within the Project and each person can receive health-social services up to 2 hours a day. The social service will be provided for 12 months within the Project. The administration of the social service "Community Care" will be carried out in two premises - municipal property, where the necessary equipment will be located. For the provision of integrated social health services, after selection, staff will be appointed, including a service coordinator, social worker, social work assistants, home medical orderlies, nurses, rehabilitators, a psychologist and a driver. Employees will be provided with training and supervision. To ensure transparency in the implementation of project activities and to promote the objectives and activities of the European Union, the European Social Fund, the HRD OP and the current project proposal, information and publicity activities will be carried out. All budget items, where applicable, will be subject to an award under the PPL. The duration of the project will cover a period of 18 months.
