The project "Patronage Care for the Elderly and Disabled People in the Municipality of Pleven" will propose the implementation of an integrated approach for the provision of integrated hourly mobile health and social services on the territory of Pleven Municipality. The project target group will be elderly people over 65 years of age, with limited or inability to self-service, as well as people with disabilities and their. Through the project, integrated mobile services will be provided for the self-service and the disabled, combining comprehensive actions to provide long-term care, including social, health and psychological, community and / or home environment ; there will be support for organizing everyday life and creating conditions for social contacts.
