Establishment of a model for patronage care in the municipalities of Simitli and Kresna

The project is implemented on the territory of two municipalities - Kresna and Simitli, which are socially high-risk and have similar demographic and infrastructure problems. Project target groups are people with permanent disabilities and adults, with chronic illnesses that for various reasons can not organize their daily lives and do not cope without support. Target groups will include people who do not have close friends able to help them, do not use social services in the community. These users due to poverty and transport restrictions are without access to social and health services in the respective municipality. Integrated cross-sectoral services will be provided after a thorough assessment of needs and the needs of each individual. An opportunity is created for part of the project users to receive rehabilitation services, self-copying training, as well as help with buying medicines, food and household products, validation of health booklets; administrative services; advice on rights and obligations. The project provides a decent life for the participants by providing integrated hourly services in a home environment and facilitated access to social and health services. A mobile team will work on the ground at the home of consumers with regard to the prevention of life-threatening illnesses, health promotion, motivational and psychological support; medical rehabilitation for handicapped people with disabilities who are without access to social services in the community. The project will work on a multidisciplinary team, which will carry out on-call duty on a schedule and in specialized consultative social-health offices in the settlements of the municipalities. For the field work, the mobile team in both municipalities will have equipped equipment and supplies, as well as a car that will reach even the most remote from the city center.
