"Patronage care in Svilengrad Municipality"

The present project proposal is related to the provision of patronage care for elderly people and persons with disabilities who have chronic diseases and permanent disabilities. The main priority of the project is the promotion of more sustainable and high quality services, which will improve the lifestyle of the people located on the territory of the Municipality of Svilengrad. The present project proposal will have an enormous social significance, because through the integrated health and social services in a home environment will enable: 1. Prevention of accommodation in specialized institutions of elderly people who, due to illness, can not organize their lives and have the need for specialized health care but do not receive them because their populated place is far from the municipal center. In most of the small settlements there are no health services, there is no regular daily public transport, which makes it difficult for the elderly, lonely people and people with disabilities from the settlements remote from the municipal center to exercise their constitutional right of access to quality health care. This makes them prefer the institutional way of ending their lives to decent old age in their home. 2. Integrated health and social services in the home environment will provide elderly people and people with disabilities with high-quality health, social, psychological and rehabilitation services at home. 3. Consumers will receive integrated health and social care services once an individual needs assessment has been prepared, where social workers, healthcare professionals and rehabilitators will determine the individual needs of each user to support and ensure a decent life for the elderly and people with disabilities.
