The project "Patronage Care in Aytos Municipality" aims to improve the quality of life and the opportunities for social inclusion of people with disabilities and the elderly people from Aytos Municipality by providing hourly integrated health and social services patronage care for the elderly and disabled, incl. with chronic illnesses and lasting disabilities in their homes. The following actions will be performed: User Selection. Assessment of needs. Concluding contracts with users or their families; Selection of patronage care staff and specialists. Team training. Providing psychological support, counseling and supervision to the team of Patronage Care in Aytos Municipality for the provision of health and social services in the home environment; Providing conditions for the provision of quality integrated health and social services in the municipality of Aytos; Providing integrated health and social services in the municipality of Aytos. Support will be given to 52 persons, each of whom can benefit from up to 2 hours of health-social services per day. A team of staff and specialists, including medical staff, dispatcher-coordinator, psychologist, driver, social worker, housekeepers, will take care of their needs. A vehicle will be bought to help the team reach the needy citizens. The services will be provided within 12 months. The total cost of the project is 168766 BGN.
