"Patronage care for the elderly and people with disabilities in the municipality of Momchilgrad"

Within the framework of the Patronage Care for the Elderly and Disabled People in the Municipality of Momchilgrad, patronage will be created at the Domestic Social Patronage - Momchilgrad. The aim of the project is to improve the quality of life of people with disabilities and the elderly over the age of 65 by providing them with access to long-term health and social care according to the specific needs of the disadvantaged person. Project budget 147 633.96 BGN Duration 18 months.The main activities of the project proposal are: - Creating conditions for the provision of "patronage care" and mobility of the service by purchasing a vehicle and retrofitting the workplaces; - Creation of a team of specialists - medical specialists, social workers, rehabilitator, laboratory technician, physician, sanitaries, driver and dispatcher, psychologist for providing hourly mobile integrated health and social services for disabled and elderly people in need; - Recruitment and selection of users of the "patronage care" service; - Providing the "Patronage Care" service; - Provide staff supervision; - Management and information and communication activities. The provision of hourly mobile services will be carried out under the program / methodology of the Ministry of Health by healthcare professionals and social services specialists, with each person in the target group receiving integrated mobile health and social services up to 2 hours per day. The project proposal is aimed at people with disabilities and elderly people over 65 with limited or inability to self-service. The implementation of the project will contribute to the implementation of the National Long-term Care Strategy, approved by Council of Ministers Decree N28/2018, the Action Plan for the period 2018-2021, the Personal Assistance Act and the Strategy for the Development of Social Services in the municipality of Momchilgrad 2016-2020, adopted by Decision №121/29.07.2016 of the Municipal Council - Momchilgrad.
