"Home Care" Center - an integrated model of patronage medical and social services for the elderly and disabled people in small rural settlements in municipality "Tundzha"

The implementation of this project is aimed at improving the quality of life and opportunities for social inclusion of people with disabilities and the elderly people by providing a network of services in the home environment, and developing adequate capacity for providing them by creating a "Home Care" Center on the territory of Municipality Tundzha. The organization of the project activities is to introduce a new model of home-based services based on "patronage care" for the elderly people and the disabled people, including chronic diseases and / or permanent disabilities, through the provision of hourly mobile integrated health and social services in home environment in small rural settlements. The activities envisaged under the project will include resource provision of the Center and the provision of hourly mobile integrated services by trained personnel to all consumers - elderly people over 65 years of age and people with disabilities as well as their families. The implementation of the project on the one hand will, provide employment for at least 20 persons of working age, including medical professionals, assisting and support staff, and on the other hand will encourage and support the deinstitutionalization process by providing the possibility of involving at least 90 users - elderly people over 65 years of age and people with disabilities who receive a complex home care. Integrated health and social services will complete the existing community services, "Home Social Patronage" and "Public Dining", specializing in the delivery of home-grown food. Offering them together will contribute to completeness in measures for independent and dignified life of older people and people with disabilities in a home environment. The implementation of the project supports and builds on similar activities implemented under projects funded under the OP "Human Resources Development" during the present and the previous programming period.
