The project is an integrated model of providing health care and social services in the home for the elderly, chronically-disabled and disabled with a view to overcoming health problems and improving their psychological and social status. Services are provided directly at home and include care, self-help, and medical activities according to the established healthcare package. The activities are aimed at elderly people over 65 with limited or inability to self-service, as well as people with disabilities and their families, people who have no relatives and who have a social need for hourly services. A large proportion of people over the age of 65 retain their ability to live independently, suffer from concomitant health problems and need medical attention. Home care will be provided by mobile teams to help individuals cope with their everyday household activities, maintaining home hygiene, body hygiene, eating, shopping, escorting as needed to a doctor or walking. The services will be provided up to 2 hours per day per person, according to the identified needs for a period of 12 calendar months. Mobile teams will include homeowners, healthcare professionals and social workers.
