The project's activities are aimed at elderly people over 65 with limited or inability to self-service, as well as for people with disabilities and their families. In addition to caring for the home, community-based services aimed at preventing loneliness and isolation and adapting to the basic needs of the elderly will also develop. People with chronic illness and permanent disability will be provided with specialized medical care in their homes. The project will create a sustainable model for providing quality services to disabled and elderly people living in the territories of the municipalities of Kotel and Tvarditsa. Integrated social-health services will be provided to achieve social impact on vulnerable groups in order to improve their standard of living. The problems of the elderly and people with different permanent disabilities will gain citizenship and help society become more responsive and responsive to their needs. The self-esteem and positive attitude of these people towards state politics in the social sphere and their belief that they are not forgotten and who cares about them will rise. Project activities will contribute to expanding social contacts and integration links between participants and target groups.
