The implementation of the project will contribute to the overall process of deinstitutionalization of the care for the disabled and the elderly through the development of a model for patronage care, providing hourly mobile integrated health and social services in their homes. Establishing a network of services in the home environment and ensuring adequate provision of those services will improve the quality of life and the opportunities for social integration of the persons in the target groups.
