Patronage care of old-age people and persons with disabilities in the municipality of Knezha

The project envisages building a model for patronage care for elderly people and persons with disabilities, incl. with chronic illness and permanent disability to provide hourly health and social services in their homes. Patronage care does not fall into the list of State-delegated activities financed by the state budget. Knezha Municipality maintains part of the services as a local activity, but the capacity of these local activities can not satisfy the needs of the needy. Knezha Municipality has not yet provided integrated health and social services in the home environment of this target group. The activities under this project are aimed at elderly people over 65 with limited or inability to self-service, as well as people with disabilities and their families, people who have no relatives and who have a social need for hourly services. Many people aged over 65, who have retained their ability to live independently, suffer from serious concomitant health problems. There is a large percentage of people with disabilities in the municipality who are not adequately supported. The project envisages providing a set of social services focused on providing care and support in the home environment. Elderly people with chronic illness and permanent disability will receive specialized medical care in their homes. Mobile service includes a variety of social, health, administrative, household activities as well as activities aimed at improving the quality of life of elderly people and people with disabilities, depending on the specific situation and constraints. Services will be provided on a per hour basis or on request to 50 users who are informed and willing to participate in needs assessment and support planning. The specific activities are aimed at stimulating the activity of the user, health care, limiting the risks in the home and the risk of institutional care dependence.
