Patronage for the elderly and the disabled in the Municipality of Rhodopi

The stage proposal Patronage care for elderly people and persons with a situation on the territory of the Municipality of Rodopi aims at: - Establishing an innovative sustainable model for providing initial mobile integrated social health service (patronage for adults and people with disabilities), building up service compensation opportunities and extending the activities imposed on the territory - for the life of the persons in the group and for the extension of the scope. Yagodovo, Rodopi - increasing the capacity of local authorities to provide mobile health-social services in the community; - Creating a minimum of 30 jobs and enabling unemployed people to move from the social economy sector to the real labor market. Within the framework of the project activities are planned for assessing the needs and selection of users of health and social services, provision of hourly mobile integrated health and social services for persons with disabilities and the elderly in the territory of the Municipality of Rodopi, provision of psychological support, counseling and training and staff supervision. Rhodope Municipality, providing psychological support, counseling and training and staff supervision. The achievement of the general and specific objectives of will lead to the provision of integrated mobile services for the target group by combining complex actions to provide long-term care, and social and health services in a home environment. All listed objectives and activities are identified in response to the investigated needs, problems and needs during the preparation of the project proposal. To support the social inclusion of these persons and to meet their needs will be assigned 1 "Manager-manager ", 1 "social worker ",4 "Nurses ",16 "domestic assistants " and 1 "Driver ". It is planned to purchase a means of transport, which is necessary to carry out the movement of specialists providing integrated health and social services to the home of the consumers.
