Patronage care in the municipality of Targovishte

With the project proposal "Patronage Care in the Municipality of Targovishte", the policies for continuing care for elderly people and people with disabilities will be implemented. A new health and social service will be created Patronage care: hourly mobile integrated health and social services for the disabled and the elderly in need. The service will be administered in a building, municipal property in Targovishte. Activities envisaged: 1. Personnel management and service delivery, 2. Provision of an integrated service "Social patronage", 3. Purchase of equipment and transport means. 4. Training and supervision. The service management staff will be composed of social workers who will draft social assessments throughout the project and administer the service (dispatchers / process coordinators). For the provision of care will be recruited homeworkers who will undergo training according to the programs developed by the Ministry of Health, Medical specialists: honey. nurse / paramedic, midwife /, rehabilitator and psychologist.We plan to buy 2 pcs. mobile computer, 1 pc. multifunctional device and 2 pcs. means of transport to reach the needy - 51 settlements in the municipality. Duration of service: 12 meters, and each person will receive a service up to 2 hours a day. The number of eligible users in the municipality of Targovishte is 118 persons. The implementation of the project proposal of Targovishte Municipality contributes to the achievement of the integrated approach in the care for the elderly and the disabled with a clear social effect and impact on the target groups - part of the whole process of deinstitutionalization of the care for people at risk, and the construction of an alternative community support through the provision of quality and affordable long-term care services
