Unit for Patronage Care - Dimitrovgrad

The main priority of the project "Unit for Patronage Care - Dimitrovgrad" is to develop and to increase the opportunities for improving the quality of life for elderly people over 65 years and people with disabilities, including also the small settlements of the municipality. For this reason a Unit for Patronage Care will be set up with a team of specialists providing hourly mobile integrated health and social services in a home environment. The employed specialists will pass training to develop appropriate capacity for providing a patronage care. The team of the Unit will implement an individual assessment to identify the specific needs and needs of every user. On the basis of this assessment, integrated hourly health and social services will be provided in a home environment. They include provision of medical services, rehabilitation and psychological support to the target groups. Lead principles during the implementation of the project activities will be their connection with the specific needs of the target groups to overcome the social isolation and to provide them with better living conditions.
