MODEL FOR NEW PATRONAGE CARE FOR ADULTS AND PERSONS WITH DISABILITIES IN MUNICIPALITY ELIN PELIN AND MUNICIPALITY OF ETROPOLE.

This project will give the municipalities of Elin Pelin and Etropole an opportunity to create a unified model for the provision of hourly mobile integrated health and social services for the elderly and disabled people in the two municipalities, Support for dignified life "and procedure BG05M9OP001-2.002" Independent life ". Within the project, hourly mobile integrated health and social services in the home environment will be provided to 106 users from both municipalities (65 for Elin Pelin Municipality and 41 for Etropole Municipality) for a period of 18 m. The provision of the services will be carried out on the basis of the specific needs of each individual representative of the target group. The services will be provided by teams of professionals with different qualifications and competencies who will be trained in advance. These teams will consist of home assistants who will provide daily support to elderly people with inability to self-service and people with disabilities, rehabilitators and nurses who will provide specialized medical care to people with chronic diseases and permanent disabilities.Two coordinators of the services will be assigned to the project and mobile social teams. Consultants who will supervise the quality of provided services will be employed as well as psychological assistants. All these will be united in a new social model - Innovative patronage for elderly people and people with disabilities - MOBILE CENTER FOR INDEPENDENT LIFE, which will be located at the Domestic social patronage of the two municipalities. Main objective: to broaden the range of social activities and services in the home environment in the form of day-care and focused health care - flexible, supportive services for more people.
