Patronage of elderly and disabled people in Ivaylovgrad Municipality

The aim of the project is to improve the quality of life and the opportunities for social inclusion of people with disabilities and the elderly people from the Municipality of Ivaylovgrad by providing services in a home environment. The integrated service provides mobile health and social services up to 2 hours per day for a period of 12 months and includes: - Assistant hourly support at home for activities where the consumer encounters difficulties (home cleaning, shopping, cooking, escorting, administrative activities, communication, etc.); - professional support by a health assistant / rehabilitator and social worker (information, counseling, rehabilitation support, depending on needs and opportunities in a home environment); - remote support in a situation of risk (by telemarking, which leads to a connection with a monitoring center and / or a visit to the user's home by a person designated by the user). The provided services will be provided to 50 users from all the settlements in the Municipality of Ivailovgrad: - Adults over 65 with limited or inability to self-service - 50 persons; - People with disabilities and their families - 8 persons; The municipality of Ivaylovgrad will award the service to the municipal social enterprise "Social patronage". 10 persons will be involved in organizing and delivering the provided services, which will undergo specialized trainings for the provision of mobile health-care services.
