The Patronage Care for the Elderly and Disabled from the Municipality of Cherven Bryag is aimed at improving the quality of life and the opportunities for social inclusion of people with disabilities and the elderly by network of services in the home environment / in the community and building of material and personnel capacity for their provision on the territory of the municipality of Cherven briag. The project provides for a complex of health-care services focused on providing care and support both in the home environment and in the community. People with chronic illness and permanent disability will be provided with specialized medical care in their homes and in the "Independent Living" scheme - a center for hourly provision of social inclusion services in the community or in the home environment. The services will be provided to 91 users on an hourly basis, on which needs will be assessed, on the basis of which the necessary support will be provided. The specific activities are aimed at stimulating the activity of the user, health care, limiting the risks in the home and the risk of institutional care dependence. The project will engage medical and social services specialists who will provide the service after specialized training under the Component 1 - a program / methodology for providing patronage care and quality of service standards. In order to perform the mobile work and in order to achieve the quality of the integrated health and social services provided in the homes of people with disabilities or persons over 65 with limited or inability to self-service, a vehicle will be purchased according to the Public Procurement Act. Consumers will be consulted and psychological support will be provided. The staff providing the service will be provided with training and supervision. The project will be implemented within 16 months, of which 12 months are provided with the patronage care service.
