Patronage care for the elderly and disabled people in Sredets Municipality

The proposed project aims at improving the quality of life and the opportunities for social inclusion of people with disabilities and the elderly in Sredets Municipality by providing a network of services in the home environment and building adequate capacity for their provision. The provision of quality hourly mobile integrated health and social services is a mechanism to prevent the risk of falling into an institution. The implementation of the project will reveal a Patronage Care Unit for provision of health and social services to elderly people and persons with disabilities in Sredets Municipality. The project fully overlaps the objectives of the Patronage Care for the Elderly and Disabled - Component 2 scheme and will respond to the need to provide assistance to target groups to organize their own lifestyle and day-to-day activities, as well as the lack of social contacts. The provision of hourly mobile integrated health and social services within the "Patronage Care Unit" will be staffed to serve those in need in the respective territory. The Unit of the Patronage Care Unit will include 9 (nine) persons. The provision of services will be based on the identified needs and problems, and the individual social assessment of the target group's representatives in need of care and inability to self-handle. We plan to serve 50 users. The implementation of the project activities will improve the quality of life and the opportunities for social inclusion of people with disabilities and the elderly by providing a network of services in the home environment and building adequate capacity for their provision.
