"Patronage care of Elderly and Disabled People in the Municipality of Devin and the Municipality of Dospat"

"Patronage care of elderly people and persons with disabilities in municipality of Devin and the Municipality of Dospat" is a project proposal that will be implemented jointly on the territories of the two municipalities. Will be provided hourly mobile integrated health and social services. The municipality of Devin and the municipality of Dospat will entrust the service to departments in the relevant municipal administration. It is planned to be created two teams – „Team for project organization and management” consisting of manager, coordinator and two accountants respectively for the municipality of Devin and municipality of Dospat and "Service management team", consisting of hired on the project dispatcher for municipality of Dospat and hired dispatcher for the municipality of Devin. For the purposes of the project here is foreseen to be hired a „Health care specialist”, specialists in the area of social activities - home sanitaries and a one dispatcher for Dospat Municipality and one dispatcher for Devin Municipality, which will provide access to health-social care,which are not available to date. It is also planned to purchase two vehicles, which will facilitate and help patronage care in the homes of users, due to the many populated and remote locations in both municipalities, most of which are difficult to have access. The target groups which will be covered are: • persons which are over the age of 65; • Persons with a valid expert decision of a labor-expert medical committee; • Persons with chronic diseases; • Persons after surgery intervention and after hospitalization, needing of short-term observation and health care and their families. The total duration of the Project is planned to be 15 months, as the provision itself of hourly mobile integrated health and social services is 12 months. It is planned to be include at least 58 persons, users of the service from different settlements in the Municipality of Devin and Dospat Municipality.
