Patronage care for deprived persons in Straldzha municipality and Bolyarovo municipality

The project "Patronage care for deprived persons in the municipality of Straldzha and Bolyarovo municipality" envisages provision of mobile health social services to 64 persons/users (41 in the territory of Bolyarovo municipality and 23 in the territory of Straldzha municipality) - elderly people over 65 years old with limitations or inability to self-service and people with disabilities and their families - for 12 months. Implementation of the project will contribute to the improvement of the quality of life and the opportunities for social inclusion as well as to the prevention of risk of institutionalization of these people through the establishment of an adequate network of quality and sustainable social and health services offered in the homes of the consumers , as well as by building appropriate (physical and staff) capacity for their provision.The planned mobile services aim to make these individuals independent and to stay as long as possible in their usual environment. They will be fully oriented to preventing loneliness, isolation and be adapted to the basic needs of consumers. Within the project hourly mobile integrated health and social services which are eligible to be provided outside a medical facility will be provided, including medical and social support, personal assistance, community-based activities, psychological support and counseling. For achieving this a Service Management Team directly involved in providing the health and social service, including psychological support and counseling will be set up, also adequate facilities to ensure the operational implementation of the project activities in both municipalities will be provided, by carrying out the necessary repairs, supply of equipment and furniture.In order to secure the possibility of providing the mobile service in the territory of Straldzha Municipality, we plan to supply a transport vehicle.
