Patronage care for elderly and disabled people in Dupnitsa Municipality

The project "Patronage care for elderly and disabled people in Dupnitsa Municipality" envisages provision of mobile integrated health and social services for 123 users - elderly people over 65 with limited self-service capacity and individuals with disabilities and their families within the territory of Dupnitsa Municipality for a period of 12 months through various activities - medical (injections, change of bandages, blood glucose measurement, rehabilitation), psychological support and counselling, communal - household assistance (accompaniment to various institutions, domestic help) and others. The complexity of this approach focuses on the following identified problems, requiring timely and proportionate actions: a) growing number of elderly people and individuals with disabilities in need of support, as well as deficits in the provision of medical services: remoteness, lack of resources, subjective individual factors - tendency to self- neglect, psychological demotivation, etc .; b) fragmentation of the provided services - social, health, education, etc. and a consequent impossibility for effective addressing of the problematic its multi-layeredness; c) negative dynamics of doctor-patient relations: lack of sustainability of the interaction, limited traceability of care and effective risk prevention. The implementation of the project will contribute to the improvement of the general quality of life with a focus on health status and social inclusion opportunities for 123 users among which elderly people with a limited self-service capacity and persons with disabilities by providing affordable and effective cross-sectoral lo ng-term services in line with the real needs of the community members. To this end, we plan to set up a team and ensure the necessary material resources for that.
