Patronage care - Sofia

The project goal is the Patronage Care Service (PCS) model to be tested in the user's home environment, under the conditions of this procedure. The project aims to provide the target group with mobile integrated health and social services in their homes in order to support their day-to-day activities, taking into account their individual needs, on a hourly basis - up to 2 hours daily. The duration of the PCS provision is 12 months as specified in the Guidelines. The service is focused on medical care supplemented by a social element. The Patronage Care Methodology (developed by the Ministry of Health under Component 1 of this procedure and approved by the Minister of Health) will be taken into account for service provision (according to degree of applicability). The project envisages serving 360 people from the following target groups: 1. According to p.13. of the Application Conditions: - "Older people over 65 with limited or inability to self-service"; - "people with disabilities" and their families; 2. According to MH's Patronage Care Methodology, the target groups are the following: - "Persons aged 65 and over; - Persons with a valid Occupational-Expert Medical Commission Decision; healthcare; - persons after being undergoing surgery and after hospitalization, in need of short-term surveillance and health care and their families. " The target number of users under this project together with the personnel costs, as well as all other eligible costs (including the costs of securing the material resources, etc.), are defined in terms of efficiency and effectiveness of the service for 12 months. Sofia Municipality will entrust the service provision to its internal unit / directorate following the procedure's Guidelines for Applicants.
