Patronage care of elderly and disabled people in Svoge municipality

The project proposal is aimed at improving the quality of life and the opportunities for social inclusion of people with disabilities and the elderly in the municipality of Svoge by providing a network of services in the home environment and building adequate capacity to provide them by introducing a patronage care model for the elderly and the disabled, incl. with chronic illnesses and lasting disabilities to provide hourly mobile integrated health and social services in their homes.The implementation of the project will be carried out by assigning the patronage activity of the Unit to the Municipality of Svoge "Center for hourly provision of services for social inclusion in the community and in the home environment". The Center was created under the project "New Opportunity for Social Support and Independent Living of the Disadvantaged People in the Municipality of Svoge", Operational Program "Human Resource Development". Services will be provided within 12 months and cover at least 88 users.This model will structure and standardize the provision of health-social patronage based on the specific training program developed by the Ministry of Health for medical specialists and social services specialists who will provide patronage care. The methodology developed by the Ministry of Health for the provision of patronage care will create unified guidelines for the specialists providing services on the territory of the municipality. The envisaged project activities are: - Selection of users and staff of services. and staff supervision - Material and technical provision of services. - Provision of hourly mobile integrated health and social services for disabled and elderly people in need. The implementation of the activities will contribute to providing a new kind of integrated health and social care services for long-term care in the community with the necessary quality and accessibility for people with disabilities and the elderly.
