"Building a model for patronage care for elderly people and people with disabilities in Rudozem Municipality and Madan Municipality"

Applying a model for the provision of hourly mobile integrated health and social services in the home environment in favor of elderly people with limited or inability to self-service or people with chronic illnesses and permanent disabilities for 12 months. Promoting their social inclusion, preventing loneliness and isolation and improving the quality of life by providing the basic needs of older people to be independent and to stay as long as possible in their usual environment. Building appropriate physical and staff capacity through training for service providers in a home environment will respond to each adult according to their needs and condition. People with chronic illness and permanent disability are provided with specialized medical care in their homes - medical care, rehabilitation and other services. For this purpose, a specialized team based on the needs of the target group will be appointed for both municipalities, such as: nurses, rehabilitators, homeowners and coordinators, who will also take the position of drivers. They will provide integrated services such as injections, changing dressings, blood glucose measurement, rehabilitation, etc., according to the needs of the target group eligible to be provided outside of a medical facility, psychological support and counseling, escorting to different institutions , household help and sanitary services, and more. Many people aged over 65 suffer from serious health problems and belong to a group of people with disabilities who have no close but have a social need for hourly services, thus protecting the public interest and responding to the needs of the population . The social inclusion of people with disabilities is of great importance and the project operation aims at this result.
