Patronage cares for adults and persons with disabilities at municipality of Chirpan

With the realization of a project "Patronage cares for adults and persons with disabilities at municipality of Chirpan" , a new type of hourly integrated health and social service will be developed for elderly people and persons with disabilities, incl. with chronic illnesses and lasting disabilities in their homes. The services will be provided by the Domestic Social Patronage - the town of Chirpan, thus expanding its activities. The overall objective of the project is to improve the quality of life and the opportunities for social inclusion of people with disabilities and the adults by providing a network of services in the home and form adequate capacity for their provision. For the provision of all services, employees are expected to be employed in the following positions: Junior Specialist "Social Activities", Psychologist, Nurse and Home Attendant. All applicants for users of social services under the project will prepare an individual needs assessment according to which each user will be provided with the services he needs. Within the services, the psychologist will provide psychological and motivational support to the users.  The nurses, if necessary and desired by consumers, will track their condition. At the user's home will measure and monitor blood pressure, blood sugar, injections and other activities in their competencies and powers. The provision of services under the project will start from the beginning of month 6. The services will be provided for 12 months. The project is expected to cover at least 61 users.
