Patronage of elderly people and people with disabilities in Dolni chiflik municipality

The following needs will be addressed: Organized social services corresponding to the growing local demand, medical and rehabilitation services aimed at elderly people and people with disabilities in Dolni chiflik municipality; multiplying good practices and fostering a culture of consumption of social services as well as a culture of behavior towards socially vulnerable groups in need by building a model of patronage care 52 persons from the target groups will be included: -elderly people over 65 with limited or inability to self-service -disabled people and their families Overall objective: Improving the quality of life and the opportunities for social inclusion of disabled and elderly people in Dolni chiflik municipality by providing a network of services in the home environment and building adequate capacity to provide them. Specific objectives: Development of a model of patronage care for the elderly and disabled Providing integrated medical care and social services in the home, psychological support and consultancy, aimed at preventing loneliness and isolation adapted to the basic needs of consumers to be independent and to stay as long as possible in their usual environment Results: Technical provision of services Trained and supervised service staff Needs assessments carried out, transparent selection of users ensuring equal access of persons from the target groups to the services Various mobile integrated health and social services, psychological support and consultancy provided in a home environment of at least 52 users and meeting their specific needs Improved quality of life of older people and people with disabilities Activities: Furnishings, equipment of premises and purchase of transport equipment Staff training and supervision Provision of hourly mobile integrated health and social services Psychological support and consultancy
