Establishment of the patronage care unit for elderly people and people with disabilities at Social Homes Patronage in Razgrad

The project envisages creation of a patronage care unit at the Social Homes Patronage in Razgrad for elderly people and disabled people as an alternative and flexible form of support in a home environment in the territory of Razgrad municipality.The main idea of the project is created for the needs of a part of the inhabitants of the municipality, for whom the social, economic and health deficit brings them difficulty and inability to cope with their everyday live.In regions with an aging population the need, the risk of social exclusion and the dependence on institutional type of care identify and prove of the need for an alternative option of care.The actions of the procedure are aimed for aged people over 65 with limited self-service and inability to self-service, people with disabilities and their families, people who do not have relatives.The implementation of the project activities is aimed at achieving a better quality of life and dignified existence of the target groups.The services are oriented towards health and social support for the prevention of loneliness and isolation and preservation of independence as long as possible.Upgrading the activities of the Social Homes Patronage and imposing an innovative model of the municipality.Expected results:within12 months,17domestic assistants,2 healthcare professionals,a medical doctor, a kinesitherapist,a psychologist and a specialist for social activities–service to a total of 200 persons at risk.Achieving a physical, mental and social wellbeing through home care, social and a psychological support,a health surveillance and a homebased kinesitherapy.Guaranteed quality and professionalism and built-up potential for such care, employees will be trained, will be provided with a professional support , a supervision and a psychological counseling.
