Patronage of elderly people and people with disabilities in the Municipality of Montana

The project "Patronage of elderly people and people with disabilities in Montana municipality" envisages the provision of mobile health and social services to 141 persons / users (elderly people over 65 with limited or inability to self-service and people with disabilities and their families) on the territory of Montana Municipality for 12 months. Implementation of the project will contribute to improving the quality of life and opportunities for social inclusion of elderly with limited or inability for self-care and disabled. Project will contribute to prevention of the risk of institutionalization of these people through the establishment of an adequate network of quality and sustainable social and healthcare services offered in consumer homes, also by building appropriate (material and staff) capacity to provide them. Mobile services aim is to make these individuals independent and stay as long as possible in their usual environment. They will be fully committed to preventing loneliness, isolation and be adapted to the basic needs of consumers. Within the project hourly mobile integrated health and social services that are eligible to be provided outside of a medical facility will be given, including personal assistance, social support and social inclusion activities, community-based activities, medical, psychological support, counseling and rehabilitation. In order to achieve this aim a management team consisting of 38 persons will be set up. It will be directly involved in the provision of the health and social service, including psychological support and counseling, as providing a suitable room to ensure the operational implementation of the project activities, by means of supplying equipment and furnishing and supplying a vehicle, which will make provision of mobile service to consumers possible not only in Montana but also in the 23 settlements in the municipality.
