Patronage care for the elderly and disabled people

The overall objective of the project proposal is to improve the quality of life and the opportunities for social inclusion of people with disabilities and the elderly by providing a network of health-social services in the home environment and building adequate (physical and staff) capacity for their provision. The specific objective of this project proposal is to develop a model for patronage care for the elderly and persons with disabilities, incl. with chronic illnesses and permanent disabilities to provide hourly mobile integrated health and social services in their homes. A model for patronage care for the elderly and the disabled, including chronic illnesses and permanent disabilities, will be built to provide hourly mobile integrated health and social services in their homes. The project activities will be aimed at elderly people over 65 years of age, with limited or inability to self-service, as well as to people with disabilities living in remote and depopulated settlements in the municipality of Sandanski, which limits their access to health and social services. Individuals will receive hourly specialized medical services, and for the elderly with chronic illness and permanent disability, specialized medical services will be provided in their homes. The project also provides for the purchase of a means of transport, which will provide access for staff to service users on the one hand and accompanying them to different health and other types of institutions when necessary. There will also be equipping suitable premises for part of the project activities.
