With the realisation of the project proposal will be implemented the measures set out in the Action Plan for the period 2018-2021 for implementation of the National strategy for long-term care, approved with Decision of the Council of Ministers № 28/19.01.2018. The project proposal is part of the overall process of deinstitutionalisation of the care for people with disabilities and elderly people and the establishment of alternative community support through the provision of quality and affordable long-term care services. The project will provide an upgrade of the established model for provision of hourly mobile integrated health and social services for elderly people in both municipalities and will be implemented by applying the already established model and integrated programmes for providing hourly mobile integrated health and social services and training of staff.
