The overall objective of the project is to improve the quality of life and the opportunities for social inclusion of disabled and elderly people from Varshets municipality by providing a network of services in the home environment and building adequate capacity for their provision. The specific objective: to develop a model for patronage care for the elderly and the disabled, incl. with chronic diseases and permanent disabilities in the municipality of Varshets for the purpose of providing hourly mobile integrated health and social services in their homes. Hourly mobile integrated health and social services for disabled and elderly people in the Varshets municipality will be provided - 50 users for 12 months.
