Patronage care for the elderly and disabled people on the territory of the municipality of Kostinbrod.

This project aims at improving the quality of life and access to services for the social inclusion of people with disabilities and over the age of 65 who are inable to self-service by overcoming the consequences of social exclusion and poverty. The project is innovative in its essence and is aimed at improving the quality of life and the opportunities for social inclusion of people with disabilities and the elderly by providing a network of services in the home environment and building adequate capacity for their provision. The project will cover at least 52 persons with chronic diseases and permanent disabilities aiming to provide hourly mobile integrated health, social and community-based services in their homes. With regard to the implementation of the direct activities and the provision of the services under the project, construction and assembly activities will be carried out on two premises. The municipality of Kostinbrod includes 13 settlements except the town, which is an administrative centre. Some of the settlements are far from the town of Kostinbrod, which in turn requires the purchase of 1 car to provide patronage care to all consumers. The services will be provided on a part-time basis by a team assigned to the implementation of the project, which will include: a coordinator of activities, a social worker, a nurse, a rehabilitator, a home assistant and a driver. Integrated services will be provided in a home environment. The integrated services complex will be provided after assessing individual needs and respecting the horizontal principles of equal opportunities and non-discrimination, gender equality. The project will be implemented in accordance with the Action Plan for the period 2018 – 2021 for the implementation of the National Long-term Care Strategy.
