Support and care for the people of Kardzhali Municipality

The overall objective of the project " Support and care for the people of Kardzhali Municipality" is to improve the quality of life and access to health-social services in home, aimed at the social inclusion of people with disabilities and the elderly in Kardzhali Municipality. The specific objectives of the project proposal are to expand the model of patronage care for people with disabilities and the elderly on the territory of the Municipality of Kardzhali; provision of hourly mobile integrated health and social services in home for disabled and elderly people and provision of material and personnel resources for provision of hourly mobile integrated health and social services in home for people with disabilities and the elderly. The target group of the project includes 157 inhabitants of Kardzhali Municipality - elderly people over 65 with limited or incapable of self-service, disabled people and their families and employees of the Municipality of Kardzhali engaged in the processes of providing health and social services. The main activities of the project are related to the selection of potential users, selection and training of a management team and personnel for provision of patronage care, and provision of hourly mobile integrated health and social services and psychological support in home environment. Under the project will be also implemented activities for information and publicity. As a result of the implementation of the project within 12 months the representatives of the target groups will receive a new health and social services, included in the patronage care. Staff will also be provided to provide the new services. The implementation of the project will contribute to expanding and improving of the patronage care through the opening of new health-social services and guaranteed access, which in turn will improve the quality of life of the target groups representatives and facilitate their social reintegration.
