"Patronage care for disadvantaged people from the municipalities of Satovcha, Borino and Sarnitsa"

This project proposal provides for the provision of a new kind of patronage care for elderly people and persons with disabilities, incl. with chronic diseases and permanent disabilities in order to provide hourly mobile integrated health and social services in their homes in the municipalities of Satovcha, Borino and Sarnitsa. The partnership aims at efficiency and cost effectiveness. The project envisages a total of 53 users from the three municipalities - Satovcha (31 users). Borino (11 users) and Sarnitsa (11 users) to receive hourly mobile integrated health and social services in their homes for 12 months. To this end, there will be employees (staff) with appropriate medical education so as to achieve the necessary quality of the provided integrated health and social services and to best meet the needs of consumers. n order to achieve the main and specific objectives set out in the project proposal, the following project activities are planned: Activity 1: Preparatory action, Information and communication measures; Activity 2: Identification and assessment of consumer needs; Activity 3: Selection of specialists providing health-social services; Activity 4: Provision of hourly mobile integrated health and social services for disabled and elderly people in the three municipalities; Activity 5: Providing psychological support, counseling;Training and supervision of staff Activity 6: Repair, furnishings and equipment of suitable workplaces, as well as the purchase of the means of transport necessary to carry out the transfer of specialists providing integrated health and social services to the home of the users. The project proposal is tailored to the needs of the region and will complement and develop the effect of already realized similar social projects.
