2. The project proposal is innovative in its essence. With its implementation, we will expand the opportunities for improving the quality of life of people with disabilities and their families, we will also increase their motivation for an independent way of life by promoting the equal opportunities of these persons for employment and labour market integration.We will implement complex actions to facilitate access to employment for vulnerable groups, including the provision of innovative social and health services. We will provide opportunities for the people who care for their disabled relatives to return to the labour market.The users of the service will have a guaranteed choice of services and will be assisted in deciding which service is best suited to their needs.We will provide innovative social-health services - a home assistant and a medical worker in their home environment, and the consumer will choose the support activities that they are in need of at the moment.The focus, duration and periodicity of activities will be determined on the basis of an assessment of service needs, the willingness of the user and the capacity of the team. We will increase the professional skills and motivation of the project staff by conducting training to provide quality, integrated, social and healthcare services.In the project proposal we plan mobility of the services that will be offered in an innovative way: integrated hourly services - social and health services in support of the persons involved in the project.
