Patronage care for elderly and disabled persons on the territory of the municipality of Saedinenie and Kuklen municipality

Project proposal aims: - Establishment of a new innovative sustainable model for the provision of hourly mobile integrated health and social services, upgrading programs in relation to existing services for the target group and expanding the activities carried out on the territory of the two municipalities; enhancing the capacity of local authorities to provide mobile health-social services in the community; creation of a minimum of 14 jobs and the possibility for unemployed people to move from the social economy sector to the real labor market; partnership to achieve cost-effectiveness and efficiency. The project envisages a total of 55 users from both municipalities - Saedinenie Municipality (37 users) and Kuklen Municipality (18 users) to receive Patronage Care for 12 months. The achievement of the general and specific objectives of the project proposal will lead to the provision of integrated mobile services for the target group by combining complex actions to provide long-term care, and social and health services in a home environment. The following project activities are planned: Activity 1: User identification and needs assessment; Activity 2: Selection of specialists providing health-social services. Concluding contracts with health-social service providers; Action 3: Provision of hourly mobile integrated health and social services for disabled and elderly people in both municipalities; Action 4: Providing psychological support, counseling; Action 5: Purchase of a means of transport necessary to carry out the transfer of specialists providing integrated health and social services to the home of consumers. The model for integrated health and social services in the home environment will provide medical and social support from medical professionals and home helpers. The services will be provided on the basis of an individual assessment in response to the needs, problems and needs of the target group.
