The overall objective of the project is to improve the quality of life and the opportunities for social inclusion of people with disabilities and the elderly by providing a network of services in the home environment and building adequate capacity for their provision in the Belitsa, Hadzhidimovo and Garmen municipalities. The specific objectives of the project are: - to develop a model for patronage care for elderly people and persons with disabilities, incl. with chronic illnesses and permanent disabilities to provide hourly mobile integrated health and social services in their homes. - Establish three working providers of social services with qualified staff and high standards to provide integrated services for people with disabilities and their families, over 65s with limited or inability to self-service, combining comprehensive actions in the direction of long-term care, incl. and social and health services in the community or in the home environment, according to the specific needs of the disadvantaged person on the basis of an individual needs assessment. The project is aimed at a total of 75 persons from the Belitsa, Hadzhidimovo and Garmen bases - elderly people over 65 with limited or inability to self-service and people with disabilities and their families. The activities that will be carried out within the project include provision of hourly mobile integrated health and social services for disabled and elderly people in need, training and supervision of staff; furnishing and equipping appropriate premises, including the purchase of means of transport for carrying out mobile work. Introduce the target groups involved in the project will increase their quality of life by accessing quality services in a home environment according to their individual needs.
