"Building an Integrated Model for Delivering Sustainable Services, incl. health and social services on the territory of the Municipality of Kirkovo"

The project proposal "Building an Integrated Model for Delivering Sustainable Services, including Health and Social Services on the Territory of Kirkovo Municipality" is aimed at providing mobile integrated health and social services in the home environment of elderly people and people with disabilities and their families, in order to ensure the effective exercise of their right to independent living and social inclusion. It is envisaged to carry out a thorough analysis and assessment of the needs of each candidate (social and health). In the municipality of Kirkovo will be provided integrated sanitary-social services by home sanitaries and specialized personnel for the provision of health services, and for these persons will be provided employment within 12 months. The provision of services will be based on the identified needs and specific needs of the representatives of the target group and the individual social assessment of each of them. Staff training and supervision are foreseen within the project. Another main activity is organizing and conducting disability therapies focusing on stimulating the creativity and self-expression of these individuals in order to attract interest and motivate the ambition of self-expression and personal growth of the most vulnerable part of society , namely people with special needs. Local social activities for social inclusion will be organized within the project, by conducting training in skills for independent living, creating conditions for social contacts, helping to participate in public life. The project will help reduce poverty and promote social inclusion through the provision of mobile integrated health and social services and the creation of new jobs.
