"Complex care for independent living"

The "Complex Care for Independent Living" project aims to improve the quality of life of disabled people and elderly people over 65 in impossibility of self-service by improving and expanding access to integrated health care services and providing comprehensive professional care in a home environment. Within the project, the social services offered in the home environment will be further developed under previous projects and schemes. There will be 1 social worker, 1 medical specialist for 14 months, 20 personal assistants, 5 social assistants, 8 home assistants for 12 months. Within the project for people with disabilities and the elderly people will be provided access to health-social care, according to the specific needs of the disadvantaged person on the basis of an individual needs assessment. The target group is disabled and adults older than 65 with no self-service The main activities of the project are: 1. Creating conditions for improving the quality and extension of the services offered by Service Center in the home environment; 2. Recruiting, recruiting and recruiting staff to provide quality social and health services to disabled persons and elderly people over 65 who are self-sufficient and unable to self-service; 3. Selection of users of services; 4. Providing social and health services to the Center; 5. Implementing motivational support and counseling of users of social and health services and supervision of the personnel providing services. The project will improve the quality of life of the elderly over 65 with incapacity for self-care and disabled people by creating conditions and support for the effective exercise of their right to independent living and social inclusion and on the other hand to ensure the employment of unemployed in working age. The project combines and implements complex actions to provide long-term care, including social and health services in the community and in the home environment.
