“CARES FOR LIFE WITH DIGNITY 5”

The aim of the project is to improve the quality of life and the opportunities for social inclusion of people from the target groups by expanding the type, scope and quality of provided integrated social and health services.Material and human capacity is being built up for the provision of these services on the territory of Iskar municipality and a complex of health, social, cultural, educational and other services in home environment and in the local community is provided for 40 consumers. After completion of the needs assessment, the necessary individual support is well planned and provided. Hourly home care is provided by a personal assistant, specialized mobile health care, rehabilitation and psychological support services are offered in home environment. The community is developing the capacity of the Home Social Patronage in Iskar, set up under the “Home Support”scheme, which offers new group integrated services aiming at improving the access to health and social inclusion. Local activities for social inclusion are offered (participation in cultural events, creating conditions for social contacts, providing support for participation in public life, etc.).The new services aim to stimulate the physical and social activity of the consumer and are focused on prevention and health care, occupational therapy, limitation of the risk of social exclusion and dependence on institutional-type care. An adapted methodology is implemented in order to provide supportive integrated cross-sectoral services in home environment and in the local community. Service quality standards are respected. For the provision of mobile health and social services of high quality in home environment and in the community, a vehicle equipped for the transportation of people with disabilities, medical equipment and rehabilitation equipment is purchased in accordance with the Public Procurement Act. Training and supervision are provided to the service personnel.
