The project aims to improve access to basic social and health services and to achieve independence and social integration for people with disabilities, as well as older people over 65 years of age without self-care. Project activities will contribute to expanding the opportunities for improving the quality of life of people from vulnerable groups and / or their families provided by previous projects, promoting equal opportunities for vulnerable groups and expanding the scope, diversity and focus of social services, provided in the community. This project proposal seeks to improve the quality of life and access to social inclusion services for people with disabilities and people over 65 years of age who are unable to self-serve, reduce poverty and promote social inclusion. The project will provide integrated social - health services that will combine complex actions towards the provision of long - term care for disabled persons and persons over 65 years of age unable to self - serve. The project will respond to the need to provide adequate support and create conditions for people with disabilities and the elderly over 65 years of age, unable to self-serve to receive quality and affordable services based on their individual needs. On the one hand, employment of unemployed persons of working age will be ensured, and on the other hand it will be possible for people with disabilities to choose the desired and necessary home help. Each disabled person will be able to use different services according to their needs and type of assistance. The project will employ "home helpers" as well as specialists to provide psychological, social and medical support to consumers. The staff who will provide the social health services under the project will be trained and will be given supervision during the implementation of the project activities.
